Payment

Health insurance may cover part or all of the treatment costs. While I am not a participating provider with any managed healthcare plan, patients may have the option of filing claims directly with their insurance carrier. Patients should check with their insurance company to determine if, and to what extent, outpatient mental healthcare is covered for out-of-network providers. Patients will receive a receipt at the end of each visit reflecting diagnosis, type of service, charges, payments, and all other necessary information needed to file a claim.

Payment in full is required at the completion of each visit. Checks, credit cards and cash are accepted. The fee structure will be discussed prior to the initial appointment.

I do not participate with Medicare, and services provided to patients who have Medicare Part B coverage will not be reimbursed by Medicare.

Because of the personal nature of this practice and the significant time allotted, I request that any cancellation of an appointment, for any reason, be made at least 48 business hours prior to your scheduled appointment. Failure to provide proper advance notice of a cancellation will result in the full fee for the scheduled appointment being charged.
